The above-mentioned customer hereby registers with the company CZECH CALL, s.r.o., Kamnářská 15, 678 01 Blansko (hereinafter called “provider”) as a leaseholder of the conference room. The customer hereby agrees with provisions aforesaid in the General terms of the provider, as rules of providing of the telecommunication services. General terms rev. 20060425 and the Price list of services are an integral part of this Customer Registration Form.
